REV: 06-10-22 SK <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Consultant will provide web-based data management services for the following modules: <br />1.All Public Services including Economic Development, (CDBG, CDGB-CV, CARES, <br />General Fund, CIP, ESG and any community service funds) at a rate of <br />$400/month <br />2.Single Family residential rehabilitation at a rate of $185/month <br />SCOPE OF SERVICES - System Maintenance <br />Under the scope of the maintenance contract, Consultant will provide: <br />Ability to store, display, and summarize individual program administration <br />information; <br />Ability to store, display, and summarize individual program contract information; <br />Ability to enter, store, display, and summarize applications, reports and fiscal <br />information to assist with management of Public Service activities; Ability to <br />transmit sub-recipient report information into HUD’s reporting system, IDIS, as <br />needed; <br />Maintain all database and report elements created under this development <br />proposal; <br />Maintain data backups and download to the City of Redwood City on a determined <br />schedule; <br />Modify database as needed to add or remove fields, or improve presentation of <br />data; <br />Modify existing reports as needed to meet City of Redwood City’s requirements; <br />Provide any applications, assessments, reports or forms to manage Community <br />Services; <br />Provide customer service to City Staff, their clients and volunteers throughout the <br />term of contract and promptly respond to request for assistance, training, or <br />database repair. <br />Limitations On Data Entered Into City Data Services <br />Confidential financial information, as determined by City, including but not limited to social <br />security numbers, birthdates and/or banking information, or any information that has the <br />potential to facilitate financial access to clients or any persons reported upon in <br />Consultant’s systems, shall not be stored on Consultant servers or held by Consultant <br />either in tangible or intangible form.
