REV: 04-07-2022 RL <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Scope of Services <br />The Contractor shall furnish emergency sewer and water damage cleaning services on <br />an as-needed basis. Upon the request of Wastewater Superintendent or their designee, <br />Contractor shall assess sewer or water damages at an identified City facility and provide <br />a written estimate of the scope of work to be completed to the Wastewater Superintendent <br />or their designee. Contractor shall not proceed with work until they have received written <br />authorization from the Wastewater Superintendent or their designee. <br />Fee schedule <br />The total not to exceed contract amount is $90,000. <br />City shall pay Contractor a flat-rate of $202.80 for a service call to assess sewer and <br />water damages at a City facility. City shall pay Contractor 1.5 times the standard rate for <br />service calls requested after 5pm or before 7am on weekdays, on weekends, and on <br />Federal holidays. <br />City shall pay Contractor for operations services on a per unit basis per the schedule <br />outlined below. City shall pay Contractor 1.5 times the hourly rate identified in the fee <br />schedule for work performed after 5pm or before 7am on weekdays, on weekends, and <br />on Federal holidays. <br />Invoices shall be billed monthly with Net 30 payments terms, in a form consistent with <br />Section 6, Compensation. <br />Operations Quantity/Type Cost <br />Water Extraction & Remediation Technician 1/hour $71.55 <br />Hazardous Waste/Mold Cleaning <br />Technician <br />1/hour $100.81 <br />Equipment setup, takedown, and <br />monitoring <br />1/hour $71.62 <br />Haul debris – per pickup truck – without <br />dumping fees <br />1/truck load $205.21 <br />Run Dehumidifier – no monitoring 24/hours $101.36 <br />Run Negative Air fan/Air scrubber – no <br />monitoring <br />24/hours $72.00 <br />Run Air Movers – no monitoring 24/hours $32.20 <br />Apply anti-microbial/disinfectant/sanitizer <br />agent <br />50/sf $50 <br />ATTY/AGR.2022.077/Palo Alto Restoration Inc.
