REV: 06-27-22 RL <br />EXHIBIT “B” <br />Workers’ Compensation and Employer’s Liability Insurance Waiver – Declaration <br />Consultant certifies business is a sole proprietor, and therefore, has elected not to be <br />covered by workers’ compensation insurance in accordance with an exception to the <br />requirement of workers’ compensation insurance coverage as provided for under Section <br />3351 (f) of the California Labor Code.
