REV: 06-27-22 RL <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Scope of Work/Tasks: <br />Consultant will provide mobile hygiene services at Fair Oaks Community Center located <br />at 2600 Middlefield Road in Redwood City two (2) days per week (Monday and <br />Wednesday) from 2:00PM until 6:00PM during the months identified in Section 3, Term. <br />Consultant will provide a driver, intake, showers, laundry, restroom, towels, body wash, <br />and essential supplies to provide comprehensive services to people experiencing <br />homelessness in Redwood City. <br /> <br /># of Sessions Cost Per Session Total <br />24 $965 $23,160 <br />Payment Terms: <br />A payment for Twenty Three Thousand, One Hundred and Sixty Dollars ($23,160) will be <br />paid to Consultant upon execution of this Agreement. <br />Upon the completion of the services, Consultant will submit a report detailing the number <br />of people served per session and the number of unduplicated people served during the <br />24 sessions.
