REV: 09-28-2022 RL <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Scope of Services <br />Contractor shall provide City with asbestos abatement services at the Schaberg Library located <br />at 2140 Euclid Avenue Redwood City, CA 94061. Contractor shall be responsible for abatement <br />of Asbestos-containing material (ACM), disposal in compliance with all applicable regulations, <br />and for submitting all applicable forms and fess require by law on behalf of the City. Services shall <br />be provided in two rooms as outlined below. <br />Task 1 - Abate ACM & ACM Mastic: Small Room (Identified as Room in Exhibit B) <br />- Mobilize all equipment and material needed for scope. <br />- Set up critical barriers along work area with negative air machines and decontamination units, <br />and exhaust to the outside of the facility. <br />- Remove and abate ACM Mastic only, from small room with ACM Mastic exposed. <br />- Remove ACM and ACM Mastic from small hallway, storage rooms, and small landing area. <br />- HEPA Vac, wipe down work area, and encapsulate properly upon completion for 3rd party air <br />clearances if needed. <br />- Properly dispose of all ACM debris generated. <br />-Submit Air Quality notifications to regulatory agencies as necessary <br /> Task 2 - Abate ACM & ACM Mastic: Big Room (Identified as Storage Area, Utility Room, <br />Closet, Hall, and Landing in Exhibit B) <br />- Remove and abate ACM & ACM Mastic from big room in locations identified per test results. <br />- Mobilize all equipment and material need for scope <br />- Set up critical barriers along work area with negative air machines and decontamination units, <br />and exhaust to the outside of the facility. <br />- HEPA Vac, wipe down work area, and encapsulate properly upon completion for 3rd party air <br />clearances if needed. <br />- Properly dispose of all ACM debris generated. <br />- Submit Air Quality notifications to regulatory agencies as necessary. <br />Fee <br />City shall pay Contractor a flat rate amount for asbestos abatement services on a per task basis <br />per the fee schedule below for a total amount of $23,325.00.
