Contract Template <$200,000 <br />May 2021 <br />REV: 09-22-22 VR <br />E. Participation and Monitoring: <br />1) Participate in the 2022 GHSMC Funding orientation meeting –in person <br />meeting or webinar – in May 2022 at which contract management guidelines <br />and reporting expectations will be reviewed. <br />2) Participate in quarterly check-ins with County staff in September, December, <br />March, and June to discuss progress, challenges, and any assistance needed. <br />3) Participate in the GHSMC “Building Healthy Equitable Communities” Forums <br />usually held twice a year. <br />4) Showcase your work by making a 10-minute oral presentation or producing a <br />handout on your project to share with participants at the 2023 Spring GHSMC <br />“Building Healthy Equitable Communities” Forum. <br />F.
