Connect unsheltered homeless individuals and households in Redwood City to housing and <br />services: <br />Provide case management services to support unsheltered homeless individuals and <br />households’ transition to permanent housing; <br />Provide housing services to support transition to permanent housing; <br />Provide or connect unsheltered homeless individuals to mental health support/services; and <br />Provide or connect unsheltered homeless individuals to substance disorder and recovery <br />support/services. <br />D. Provide resources for unsheltered homeless individuals and households in Redwood City: <br />Provide or connect unsheltered homeless individuals and/ or households to hotel respite <br />resources administered by the City; <br />Connect unsheltered individuals and/or households to housing subsidies administered by <br />the City; <br />Provide supplies or other items to assist unsheltered homeless individuals or households <br />with meeting their basic needs and to build rapport (food, gift cards, socks, glasses, etc.); <br />Provide transportation assistance to assist unsheltered homeless individuals or households <br />with connecting to services. <br />E. Document and track contacts and services provided: <br />Document and track all inquiries, responses, and case management plans in the San Mateo <br />County Clarity HMIS (Homeless Management Information System) in a timely manner. <br />Utilize additional, complimentary tools that assist with documentation and tracking <br />Objectives: <br />During the term of the Agreement, Consultant will report on the Performance Measures <br />identified below.
