REV: 09-21-2022 SK <br />FUNDING AGREEMENT FOR HUMAN SERVICES <br />FINANCIAL ASSISTANCE PROGRAM <br />BETWEEN THE CITY OF REDWOOD CITY AND <br />SERVICE LEAGUE OF SAN MATEO COUNTY <br />THIS AGREEMENT for Human Services Financial Assistance Program <br />(“Agreement”), made and entered into this day of , 2022, <br />by and between the CITY OF REDWOOD CITY, a charter city and municipal corporation <br />of the State of California (“City”), and Service League of San Mateo County, a California <br />nonprofit corporation (“Contractor”). Collectively, the City and Contractor will be referred <br />to as the “Parties.” <br />RECITALS <br />A. WHEREAS, Contractor provides a fully licensed facility to administer a <br />comprehensive, structured, residential substance use disorder (SUD) and mental health <br />treatment (“Program”); and <br />B. WHEREAS, Contractor has requested financial assistance in order to <br />conduct the Program for residents of City during the fiscal year commencing July 1, 2022, <br />and ending June 30, 2023; and <br />C. WHEREAS, City has reviewed the request and desires to allocate to <br />Contractor the sum of Ten Thousand Dollars ($10,000). <br />NOW, THEREFORE, the Parties hereby agree as follows: <br />1. FINANCIAL ASSISTANCE. City shall allocate to Contractor the sum of Ten <br />Thousand Dollars ($10,000) for exclusive use by Contractor during the fiscal year <br />commencing July 1, 2022, and ending June 30, 2023, to be used by Contractor solely for <br />the purposes described in Section 2. <br />2. USE OF FUNDS. Contractor shall use the funds provided pursuant to <br />Section 1 solely for the Program purposes described in Exhibit “A” (Contract Objectives) <br />attached hereto and by this reference incorporated herein. <br />3. CHANGES TO PROGRAM. No changes in the Program as described in <br />Exhibit “A” to this Agreement shall be made without the prior written consent of City <br />Manager. <br />4. PAYMENTS. Contractor will keep detailed and accurate records of all <br />expenditures made and expenses incurred which are funded under this Agreement. <br />Payments of funds allocated pursuant to Section 1 shall be made to Contractor within 30 <br />calendar days after this Agreement is fully signed and executed. <br />ATTY/AGR.2022.355/Service Leagie of San Mateo County (Page 1 of 10) <br />27th December
