REV: 09-21-2022 SK <br />EXHIBIT A <br />CITY OF REDWOOD CITY <br />Service League of San Mateo County <br />FISCAL YEAR 2022-2023 <br />Agency Information <br />Agency Name:Service League of San Mateo County <br />Address:3789 Hoover Street <br />City, State, Zip:Redwood City, CA 94063 <br />Activity/Program Title:Hope House Residential Program for Women & Infants <br />Quantitative Goals <br />Description of Services to be Provided: <br />The Service League's Hope House provides temporary shelter, food and other emergency and life- <br />sustaining services for an average of 50 individuals and their infants annually. Hope House is fully <br />licensed to administer a comprehensive, structured, residential substance use disorder (SUD) and <br />mental health treatment. Funding will help cover the costs of materials and supplies needed to <br />adequately serve this vulnerable population, which has been even more challenging than usual. <br />The staff has had to strategize to ensure continuity of programs (instructor-led classes), essential <br />appointments (clinic and other crucial visits) in the safety manner possible by implementing remote <br />access wherever feasible utilizing Zoom. <br />Primary Quantitative Objectives: <br />Service League will serve 50 incorporated Redwood City persons in 50 households. <br /> <br />NOTE: The grant performance period begins July 1, 2022 and concludes June 30, 2023. <br />ATTY/AGR.2022.355/Service Leagie of San Mateo County (Page 10 of 10)
