Funding from HSFA will support the core nutritional and wellness services <br />for older adults age 60+ at the Fair Oaks Adult Activity Center in Redwood City. <br />These meals are provided as grab and go meals. Since COVID-19 Fair Oaks <br />Adult Activity Center is closed to in-person services, the model has been <br />adjusted to support the long-term impact of the health and wellness of <br />vulnerable, primarily low-income older adults. <br />Primary Quantitative Objectives: <br />Contractor will serve 220 incorporated Redwood City persons in 220 <br />households. <br /> <br />NOTE: The grant performance period begins July 1, 2022 and concludes June 30, 2023. <br />ATTY/AGR.2022.367/Peninsula Family Services (Page 10 of 10)
