20 EAP Services Agreement <br />o 6 Hour Course -This option provides six (6) hours (2 hours self-directed <br />pre-work + 4 hours of instructor led training) of standard Mental Health <br />First Aid curriculum. Fee includes all instructor fees and customization. <br /> <br />o 4 Hour Course - This option provides four (4) hours of standard Mental <br />Health First Aid curriculum. Fee includes all instructor fees and <br />customization. <br /> <br />Cancellation: Group agrees to provide Plan with at least thirty (30) days advance notice of <br />cancellation of a scheduled Mental Health First Aid course. If Group cancels for any reason <br />within 30 days from the scheduled training date, Group will be responsible for cancellation <br />fees as follows: <br /> <br /> 50% of the total fee if cancelled within 15-30 days prior to the scheduled date of <br />training. <br /> 100% of the total fee if cancelled within 0-14 days prior to the scheduled date of <br />training. <br /> <br />Rescheduling: If Group reschedules for any reason within 30 days from the training date, Group <br />will be responsible for the rescheduling fees of 25% of total fees. <br />REV: 03-10-23 MI <br />ATTY/AGR.2023.039/Aetna (EAP Services Agreement) (Page 21 of 42)
