Plan’s Public Policy Committee <br />Plan has established a Public Policy Committee that includes, among others, Members of <br />Groups that have contracted with Plan for Benefits. This committee meets quarterly and <br />Plan’s Board of Directors reviews the reports and recommendations of the committee. Any <br />Member desiring more information about this committee should contact Plan at 1-800-342- <br />8111. <br /> <br />E. Term and Renewal Provisions <br />The initial term of the EAP Services Agreement is thirty-six (36) months. Thereafter the <br />agreement is automatically renewed for successive twelve (12) month periods, subject to <br />the termination provisions contained therein. <br />F. Important Information about Organ and Tissue Donations <br />Organ and tissue transplants have helped thousands of people with a variety of problems. <br />The need for donated organs, corneas, skin, bone and tissue continues to grow beyond the <br />supply. Organ and tissue donation provides you with an opportunity to help others. Almost <br />anyone can become a donor. There is no age limit.
