REV: 03-01-23 RL Page 19 1. County may add, adjust, or modify all Grantee Performance Measures, goals, and targets. The County shall provide Grantee with prior written notice of any such modifications. 2. Grantee shall cooperate with and participate in Site Review/Compliance Visits with County designated staff. County will attempt to provide Grantee at least two weeks prior notice of any such Site Review/Compliance Visit(s), unless there is an urgent programmatic need, as determined in County’s sole discretion, to expedite the process. 3. Grantee shall cooperate with and participate in Project evaluations, homeless outreach evaluations, and analysis of the homeless system conducted by County, as well as technical assistance provided by County or County contracted provider such as Homebase. B. Reports 1. Grantee shall submit all reports required under the Agreement (including those specified in Section 6 and Exhs. B and C) electronically to Nikki Gilman, ngilman@smcgov.org, or designee. a. Grantee’s monthly performance reports are due by the last day of the month for the previous month’s service. Reports will include the following: i. Performance measure report (results for Performance Measures for the current month and for Project to-date). ii. A brief narrative describing trends, successes, and challenges during the reporting period. b. Grantee’s annual report is due within 30 days after the end of the fiscal year and shall include: i. Narrative information on the impact of homeless outreach and engagement services throughout the prior service year. ii. Annual results for each Performance Measure. c. Grantee’s final report is due within 30 days of the end of the Agreement term and will include: i. Narrative information on the impact of homeless outreach and engagement services throughout both the entire service year and term of the Agreement. ii. Both annual and total results for each Performance Measure. d. Grantee shall provide County with annual financial audit statements in accordance with generally accepted government auditing standards within nine months of fiscal year end (for each year of Agreement). 2. County may request additional reports or data from Grantee and/or retrieve reports from Clarity HMIS to understand client requests, services, and outcomes. County will provide Grantee with at least two weeks prior notice of a request for additional reporting beyond that specified in this Agreement, unless there is an urgent programmatic need, as determined in County’s sole discretion, to expedite the data/report.
