REV: 06-30-23 RL <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Consultant will provide the following Services: <br />Peer-to-Peer Support Groups for Youth: 10-week youth peer mentorship groups <br />that address the substance abuse crisis by providing education and mentorship to <br />middle school students. <br />Peer-to-Peer Support Groups for Parents: 10-week parent peer mentorship groups <br />that address the substance abuse crisis by providing education and mentorship to <br />parents of middle school students. <br />Deliverables: <br />Participating youth and parents will be educated about substance use indicators in <br />friends, family and community, and the physical impact of alcohol and other drugs. <br />Participating youth and parents will be able to recognize the connection between <br />substance use and depression and/or anxiety and other behavioral issues. <br />Participating youth and parents will be taught how to implement refusal skills and <br />teach others to make better choices. <br />Participating youth and parents will be taught how stress is reduced without using <br />alcohol or other drugs. <br />Participating youth and parents will advocate for people struggling with addiction, <br />connecting to resources, and lead a respectful, trusting, empathetic, collaborative <br />and mutual partnership in helping them achieve a healthier lifestyle. <br />Fees: <br />Upon execution of the Agreement for Services, Consultant will invoice City and be <br />paid an initial installment of $20,000. <br />Upon completion of all Services, Consultant will be paid a second installment of <br />$19,755.22, pursuant to an invoice consistent with Section 6, Compensation, that <br />will evidence progress in achieving defined deliverables. <br />ATTY/AGR.2023.163/El Centro de Libertad (Youth Peer Mentorship Program) (Page 11 of 11)
