CACHI Milestones Checklist 2023 New ACH 9 / 9 Q14 A. The ACH identifies, collects, synthesizes and reports data/information to monitor and communicate, through a dashboard or other mechanisms, progress regarding ACH assets and infrastructure (e.g., Wellness Fund), outcomes (e.g., selected health condition), equity and overall impact. The group of organizations identify relevant data sources to inform the goals and priorities.
