REV: 02-28-24 VR <br />AMENDMENT NO. 1 TO AGREEMENT <br />MENTAL HEALTH ASSOCIATION OF SAN MATEO COUNTY <br />Spring Street Shelter <br />This Amendment No. 1 (the “Amendment No. 1”) is entered into and effective as of <br />__________________, 2024 by and between the City of Redwood City, a charter city and <br />municipal corporation of the State of California (“City”), and Mental Health Association of <br />San Mateo County, a California nonprofit corporation (“Subrecipient”“) (collectively, the <br />“Parties”). <br />RECITALS <br />A. The Parties previously executed that certain Agreement, dated as of <br />November 22, 2023, (the “Original Agreement”). <br />B. The Parties have agreed to replace Section 2 (Federal Award Identification) <br />of the Original Agreement with the revised Section 2 as shown below. <br />NOW, THEREFORE, in consideration of these recitals and the mutual covenants <br />contained herein, the Parties agree as follows: <br />1. Section 2 (Federal Award Identification) of the Original Agreement is hereby <br />deleted and replaced with the following: <br />“2. FEDERAL AWARD INDENTIFICATION. In accordance with Title 2 CFR (Code of <br />Federal Regulations) Section 200.331(a) (Requirements for Pass-Through Entities), the <br />following notification is to designate the use of federal funds in this Subaward: <br />i. Subrecipient name Mental Health Association of San Mateo <br />County <br />ii. Sub recipient’s unique entity <br />identifier (Sub recipient DUNS <br />number) <br />96-632-6642 <br />iii. Federal Award Identification <br />Number (FAIN) <br />B-23-MC-06-0014 <br />iv. Federal Award Date July 1, 2023 <br />v. Subaward Period of Performance July 1, 2023- June 30, 2024 <br />vi.
