REV: 12-20-23 MI <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />SCOPE OF SERVICES <br />Contractor shall provide City with overhead door maintenance services for City owned <br />facilities on an as-needed basis at the request of the Assistant Public Works Director or <br />their designee. Services may include: <br />•Testing door and motor operability <br />•Testing safety edges <br />•Testing remotes and transmitters <br />•Adjusting limits and chain slack <br />•Adjusting logic board settings and timers <br />•Adjusting idler pulleys <br />•Adjusting trolley drive shaft assemblies <br />•Loosening and tightening motors and chains <br />•Resetting take-up reels and rollers <br />•Inspecting drums, cables, bearings, hinges, rollers, and set screws; lubing when <br />necessary <br />Contractor shall provide City with written estimates for all as-needed work requests and <br />obtain approval from the Assistant Public Works Director or their designee prior to <br />performing work. <br />Contractor shall respond to emergency service calls within one hour of receiving <br />notification from City. <br />FEE <br />City shall pay Contractor on a time and materials basis per the fee schedule below in an <br />amount not to exceed Ninety-Nine Thousand Dollars ($99,000). Invoices shall be billed <br />monthly with Net 30 payments terms, in a form consistent with Section 6, Compensation. <br />ATTY/AGR.2023.317/R & S Erection North Peninsula, Inc.
