REV: 12-14-23 SK <br />EXHIBIT “A” <br />CITY OF REDWOOD CITY <br />FISCAL YEAR 2023-2024 <br />Agency Information <br />Agency Name:Fresh Lifelines for Youth, Inc. <br />Address:568 Valley Way <br />City, State, Zip:Milpitas, CA 95035 <br />Activity/Program Title:Law and Leadership Program for Redwood City Youth <br />Quantitative Goals <br />Description of Services to be Provided: Contractor will provide its Law and Leadership <br />Programs to at risk and system-involved Redwood City youth. Youth will participate in a 12- <br />week Law program. Key priorities of the Law Program are that youth build life skills and <br />Social Emotional Learning (SEL) skills—assets associated with future success and reduced <br />system involvement. Upon completion of Contractor’s Law Program, youth who need <br />additional services to remain free of system involvement and become productive, healthy <br />adults can transition into Contractor’s Leadership Program. During the 10-month program, <br />Contractor will provide youth with individualized coaching/case management from a FLY <br />Case Manager (CM) as well as systems navigation support from a FLY Career Pathways <br />Navigator (CPN). <br />Primary Quantitative Objectives: Contractor will serve 24 incorporated Redwood City <br />persons in 24 households. <br />NOTE: The grant performance period begins July 1, 2023 and concludes June 30, 2024. <br />ATTY/AGR.2023.314/Fresh Lifelines for Youth, FLY (Law and Leadership Program for Redwood City Youth) (Page 10 of 10)
