REV: 07-22-24 LF EXHIBIT “A” SCOPE OF SERVICES Consultant’s Executive Director will participate in monthly Community Collaborative for Children’s Success (CCCS) Core Team meetings Consultant will advance the values and aims of the CCCS plan by actively participating in activities outlined in the plan and attending the monthly CCCS Neighborhood Action Team (NAT) meetings Consultant will serve as an ambassador for the CCCS Core Group Consultant will co-create CCCS’ vision and policies along with the CCCS NAT to Redwood City Together Leadership Council, and City and San Mateo County representatives Consultant will assist the CCCS Core group with review of grant proposals and offer feedback and support Consultant will co-create and uphold policies as a member of CCCS Team Consultant will consider establishing forum for cross-sector collaboration on community matters, provided that the hosting organization’s primary activities permit the allocation of sufficient resources (space, personnel, and equipment) to facilitate such collaboration effectively Consultant will engage in quarterly professional development trainings hosted by Redwood City Together with other partners Consultant will support with data collection such as surveys that will inform the movement of the core group Consultant will attend annual CCCS retreat Consultant will measure activity on the chosen two aim statements from the CCCS plan indicators o The CCCS Core Team will increase awareness of early childhood development and education opportunities through 1-2 events by July 2024 in collaboration with the school district, library, early childhood education programs, etc., for families with children prenatal to 5 years old in Redwood City and North Fair Oaks. o The CCCS Core Team will support local early childhood providers such as Family Connections with guidance and outreach to expand programming focused on early childhood development by July 2024 for the Redwood City and North Fair Oaks communities. Activities: Will provide support with community survey dissemination Success will be measured by Increasing o the numbers of families participating o improved outcomes for those families Consultant will submit an end of the year report on progress on chosen aim statements ATTY/AGR.2024.114/Family Connections (CACHI Subcontract) (Page 11 of 12)
