REV: 07-22-24 LF <br />EXHIBIT “A” <br />SCOPE OF SERVICES <br />Consultant’s Executive Director will participate in monthly Community <br />Collaborative for Children’s Success (CCCS) Core Team meetings <br />Consultant will advance the values and aims of the CCCS plan by actively <br />participating in activities outlined in the plan and attending the monthly CCCS <br />Neighborhood Action Team (NAT) meetings <br />Consultant will serve as an ambassador for the CCCS Core Group <br />Consultant will co-create CCCS’ vision and policies along with the CCCS NAT to <br />Redwood City Together Leadership Council, and City and San Mateo County <br />representatives <br />Consultant will assist the CCCS Core group with review of grant proposals and <br />offer feedback and support <br />Consultant will co-create and uphold policies as a member of CCCS Team <br />Consultant will consider establishing forum for cross-sector collaboration on <br />community matters, provided that the hosting organization’s primary activities permit the <br />allocation of sufficient resources (space, personnel, and equipment) to facilitate such <br />collaboration effectively <br />Consultant will engage in quarterly professional development trainings hosted by <br />Redwood City Together with other partners <br />Consultant will support with data collection such as surveys that will inform the <br />movement of the core group <br />Consultant will attend annual CCCS retreat <br />Consultant will measure activity on the chosen two aim statements from the <br />CCCS plan indicators <br />o The CCCS Core Team will increase awareness of early childhood <br />development and education opportunities through 1-2 events by July 2024 in <br />collaboration with the school district, library, early childhood education programs, <br />etc., for families with children prenatal to 5 years old in Redwood City and North <br />Fair Oaks. <br />o The CCCS Core Team will support local early childhood providers such as <br />Family Connections with guidance and outreach to expand programming focused <br />on early childhood development by July 2024 for the Redwood City and North <br />Fair Oaks communities. <br />Activities: <br />Will provide support with community survey dissemination <br />Success will be measured by <br />Increasing <br />o the numbers of families participating <br />o improved outcomes for those families <br />Consultant will submit an end of the year report on progress on chosen aim <br />statements <br />ATTY/AGR.2024.114/Family Connections (CACHI Subcontract) (Page 11 of 12)
