<br />57 <br /> <br />immediate supervisor, the Shift Battalion Chief or <br />Personnel Officer. <br /> <br />18.8.2.2 At least thirty (30) days prior to the beginning of the <br />pregnancy leave, the employee shall submit to the <br />Personnel Officer the specific date she intends to <br />begin the leave, accompanied by her physician's <br />written statement attesting to the employee's ability <br />to continue performing the full schedule of her duties <br />and responsibilities. She shall continue on active duty <br />until the specific date, providing she performs the full <br />duties and responsibilities of her position and <br />furnishes additional health statements from her <br />physician upon reasonable request. In the event the <br />employee is unable to perform the essential duties <br />and responsibilities of her position, she shall be <br />assigned to light duty until the specific leave date, and <br />shall continue to furnish additional health statements <br />from her physician upon reasonable request. <br /> <br />18.8.2.3 Prior to the establishment of a specific date for return <br />to duty, the employee shall submit to the Personnel <br />Officer a notice of intention to return to duty, <br />accompanied by her physician's statement certifying <br />that the employee is medically qualified to assume <br />regular duties and responsibilities. <br /> <br />18.8.2.4 The Personnel Officer or designee may designate the <br />specific beginning and ending dates within the pay <br />period requested by the employee to meet the needs <br />of the employee and the City. <br /> <br />18.8.3 Following the pregnancy leave of absence, the employee shall be <br />returned to her original position. If that position has been <br />eliminated, applicable layoff and reemployment rules shall apply. <br /> <br />18.8.4 A pregnancy leave, absent physical disability, is granted without pay <br />for the duration of leave. Employees may use paid leave accruals in <br />accordance with City policy, as periodically amended. The employee <br />may elect to continue medical and dental insurance coverage for up <br />to one (1) year during this leave. For the period the worker is on <br />approved, unpaid, non-statutory leave, coverage will be at her own <br />expense. <br /> <br />6.A. - Page 68 of 99 <br />75
