RE: 06-27-24 MI <br />EXHIBIT “A” <br />SCOPE OF SERVICES, PAYMENT TERMS AND INSURANCE <br />I. SCOPE OF SERVICES <br />As-Needed Supplies <br />The City shall purchase various personal protective equipment and safety items, as well <br />as other maintenance, repair, and operations supplies, on an as-needed basis as <br />requested by authorized City employees, The City does not guarantee, nor agrees to <br />purchase a minimum or maximum quantity of Goods. <br />Vendor shall provide the Goods from the categories outlined in Section II of Exhibit A <br />through the following methods: <br />•Shipping to 1400 Broadway, Redwood City, CA or other location as determined by <br />City staff <br />•Will-call pick-up in-person at 1546 Montague Expy, San Jose, CA 95131 <br />•Vending machine onsite at Public Works Services (see “Vending Machines” <br />section below). <br />Vending Machines <br />Vendor shall provide three vending machines onsite at Public Works Services (located at <br />1400 Broadway, Redwood City, CA), which include: <br />•One (1) FAST 3000 Machine <br />•Two (2) 18-Door Lockers <br />The City shall provide physical space for these lockers and may request to move them as <br />needed. Vendor shall maintain full ownership of these machines and be responsible for <br />all maintenance and repair. Vendor shall also retain the title and all risk of loss and <br />damages with respect to the products stored in the vending machines until the City <br />dispenses the item. Vendor shall coordinate with City to determine what products shall <br />be included in the vending machines, and the City may update those items, as needed. <br />Vendor shall manage inventory for these machines and also provide online reporting of <br />items dispensed. Vending machines shall dispense items selected by staff on a per unit <br />basis. Vendor shall invoice the City only after an item has been dispensed from the <br />vending machine. <br />II.
