REV: 11-20-24 MI <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Scope of Services: <br />Contractor shall provide carpet cleaning services on an as-needed basis, at the request <br />of the City. If requested by the City, Contractor must be able to provide service on nights <br />and weekends, in multi-story facilities, and possess a hose bib key to connect to and <br />operate external water sources at project facilities. The City does not guarantee any <br />minimum or maximum quantity purchased. <br />Carpet cleaning services shall include cleaning of area rugs, entry mats, and standard <br />carpeted areas. With all carpet cleaning services, Contractor shall be required to provide <br />pre-conditioning treatment, which entails stain removal and spot cleaning in all project <br />areas. Carpet cleaning shall include shampooing, extraction, and use of air movers to <br />ensure project areas are dried to the City’s satisfaction. <br />Fee: <br />City shall pay Contractor on a cost per square foot basis as outlined in the table below in <br />an amount not to exceed Ninety-Nine Thousand Dollars ($99,000). Contractor will only <br />issue an invoice once services have been completed. Payment will be made within thirty <br />(30) days receipt of an accurate invoice in a form consistent with the requirements of <br />Section 6, Compensation. <br />Carpet Cleaning Services <br />Cost per square foot (sq. ft.) $0.25/sq. ft. <br />ATTY/AGR.2024.218/Lewis Carpet Cleaners, Inc.
