REV: 02-20-25 LF <br />EXHIBIT “A” <br />Program Objectives, Scope of Services & Quantitative Goals <br />Program Year 2024/2025 <br />Subrecipient Information <br />Subrecipient Name:StarVista <br />Address: 639 Douglas Ave <br />City, State, ZIP: Redwood City, CA 94063 <br />Project Name: Daybreak Shelter <br />Program Objectives <br />Project will address Redwood City’s Consolidated Plan 2023-2027 priorities <br />and goals as follows: <br />Priority: Supporting Persons Experiencing Homelessness- Making <br />homelessness short, rare and nonrecurring by supporting the work of emergency <br />shelters and funding other basic needs. <br />Goal: Homeless Services: Provide high-quality, professional services in all <br />temporary shelters and comprehensive outreach services designed to directly <br />connect individuals to all appropriate housing and services. <br />Scope of Services <br />Description of Services to be Provided: <br />Daybreak Shelter provides transitional housing and independent living skills <br />training for youth experiencing homelessness aged 16-21 to support them in <br />achieving self-sufficiency and exiting to permanent housing. <br />Project Activities: <br />Daybreak Shelter provides youth with safe and stable housing, food, <br />independent living skills training, resource linkages, and other comprehensive <br />services designed to support a successful transition from unstable housing to <br />healthy, positive living environments and self-sufficiency. By providing shelter, <br />healthy meals, and trained counselors, Daybreak Shelter empowers youth to <br />break the cycles of homelessness, poverty, and addiction they face to get back <br />on track with their lives and goals. <br />Primary Quantitative Goals: <br />Daybreak Shelter will provide transitional housing and independent living skills <br />training for 8 homeless Redwood City youth aged 16-21. <br />NOTE: Performance will be measured by program objectives and quantitative goals.
