REV: 02-05-25 LF <br />Exhibit “A” <br />Scope of Services <br />Consultant will support 13-15 Youth Promotores that canvass to share resources <br />that improve health, wellness and equity in the Redwood City and North Fair Oaks <br />community. <br />Consultant, together with Redwood City Together will develop the next generation <br />of community leaders. Through active participation in events and food distribution <br />efforts, the program fosters a deeper understanding of community needs while <br />cultivating servant leadership skills. <br />Consultant, together with the Youth Promotores will assist families with registering <br />for various community programs, including Medi-Cal, the Mariposa Parks Pass, <br />NovaWorks, and Commute.org, and engage in one-on-one outreach to enhance <br />access to local resources and citywide events and partner closely with Fair Oaks <br />Community Center to reach our hardest to reach families and individuals. <br />Consultant will train Youth Promotores to help build interpersonal skills and receive <br />professional development training in areas such as financial literacy and college <br />readiness in the community of Redwood City. <br />By June 30, 2025, Consultant, along with the Youth Promotores plan to reach over 10,000 <br />community members and continue moving the needle on signing up families for Medi- <br />Cal, County resources and job opportunities in addition to sharing citywide community <br />events. <br />ATTY/AGR.Amend/No. 1/Friends for Youth (Promotores Outreach FY24-25) (Page 4 of 4)
