REV: 03-11-25 LF <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Scope of Services: <br />Contractor shall provide maintenance and testing services for the Carbon Monoxide (CO) <br />detection system located at the City-owned Jefferson Avenue Parking Garage located at <br />950 Jefferson Avenue. Contractor shall perform the tasks as outlined below. <br />Task 1 (Controller and Sensor Replacement) <br />•Replacing one INTEC controller and 10 sensors for the CO detection system <br />•Removing obsolete controller and CO sensors <br />•Installing new parts for, reprogramming, and testing the CO detection system <br />• All test kits and test gases shall be supplied and paid for by the <br />Contractor <br />Task 2 (Bi-Annual Preventative Maintenance) <br />•Programming and testing of control panel operation <br />•Troubleshooting, evaluating, and reporting all issues at the site <br />•Testing and calibrating all 36 CO Sensors, and cartridges (if needed) <br />•Replacing failed CO sensors and cartridges (if needed) <br />•Providing detailed report of findings and recommendations <br />Task 3 (Extra Work As-Needed) <br />Contractor shall respond to calls for unscheduled work (“Extra Work”) from the City as <br />soon as possible but no later than two business days. <br />Contractor shall provide any of the services listed below at the request of the Public <br />Works Facilities Superintendent or their designee on a time and material basis with a <br />percent mark-up on materials as agreed upon by the City and Contractor. <br />•Programming and testing of control panel operation <br />•Troubleshooting, evaluating, and reporting all issues at the site <br />•Testing and calibrating CO sensors, and cartridges <br />•Replacing failed CO sensors and cartridges <br />•Providing detailed report of findings and recommendations <br />Fee: <br />City shall pay Contractor on a per task or time and materials basis as outlined in the table <br />below in an amount not to exceed Ninety-Nine Thousand Dollars ($99,000).
