GENERAL AMENDMENT FORM CITY ATTORNEY APPROVED VERSION REV: 10-10-24 LF Page 4 of 5 Exhibit “A” SCOPE OF SERVICES AND FEE Scope of Services: Contractor will perform trucking and hauling services on an as-needed basis for the City, which consist of the following: Normal Work •Furnish materials, trucks, and drivers to pick up or deliver directed materials to specified locations. Examples of specified locations would be the Public Works Municipal Service Center (MSC), an active job site, or proper disposal site. •All materials supplied to the City shall comply with the City's Engineering Standards (http://www.redwoodcity.org/departments/community- development-department/engineering-transportation/engineering/engineering- standards). If City's Engineering Standards are not available, materials shall comply with applicable State of California standards, such as Caltrans Construction Contract Standards, or other standard as requested by the City. •Deliver or pick up materials in a timely manner. Materials shall be delivered in usable condition. The material picked up shall be the type and amount of material designated by the City for the particular job in process. •On each trip to the MSC, call the designated City staff member to confirm arrival and departure times. •Delivery tags for all materials picked up and / or delivered, as well as record tags or time sheets for the work performed, shall be provided to the City on the day the delivery or pick up was performed. •Vehicles entering the MSC property shall follow all posted speed limits, directions of travel, correct entrance and exit use, and parking and loading zones. •Dispose of all materials in accordance with all State, Federal, and Local laws. Emergency Response •Provide emergency response 24 hours a day, 7 days per week (including holidays), on a "24/7" basis. •Maintain a single, local telephone contact number where an on-call representative can be reached at all times (24/7). Provide the telephone number to all persons designated by the City. •Services requiring response within 3-hours shall be classified as an emergency response.
