REV: 06-27-25 MI <br />“ ”EXHIBIT A <br />SCOPE OF SERVICES AND FEE <br />Consultant will perform all services onsite at a City facility. Consultant will perform an <br />initial assessment on all sworn Public Safety Department employees, including <br />firefighters, police officers, etc. Their first session will include: Functional Movement <br />Screening, Range of Motion, and Strength Testing. Consultant will use the information <br />from the initial assessment to customize a treatment plan for each individual. Consultant <br />shall provide physical therapy treatment services on an as-needed basis to the City. The <br />City will be invoiced on an hourly basis for actual services provided. The total hours billed <br />shall not exceed 449 hours for this contract. <br />Consultant shall perform the services outlined above on an hourly basis at the rate below. <br />Hourly Rate: $200 per hour <br />Total Not-to-Exceed Contract Amount: $ 99,900 <br />ATTY/AGR.2025.153/Worklife (On-Site Physical Therarpy Services) (Page 11 of 11)
