List all spare parts, and a separate list of recommended spare parts and <br />quantities to be provided, including: <br />a. Part name. <br />b. Part number. <br />c. Unit price. <br />D. Exceptions and Substitutions: <br />1. Complete and submit an all-inclusive Specification Compliance List <br />(including substitutions), identifying any substantive non-compliance with <br />specification requirements. <br />2. Substitutions: It is recognized that there are variations in equipment <br />between manufacturers. Where functional performance, features or quality <br />of system varies materially from that specified, submit a Request for <br />Substitution identifying substitution being proposed. This submittal may be <br />accompanied by catalog sheets, brochures, and technical specifications of <br />the proposed system. <br />E. Project Schedule: Detailed schedule of project requirements including milestones <br />for fabrication, delivery, installation, training, and testing. <br />F. List manufacturer of each primary component of system. <br />G. Contractor Information <br />1. Provide the following information: <br />a. Name of company. <br />b. Brief company overview including a description of the company <br />structure, and a statement on whether the company is a private or <br />public entity. <br />c. Years in business. <br />d. Number of employees. <br />e. Annual sales volume. <br />f. List of key accounts. <br />g. Name and contact information (address, email, and phone number) of <br />Principals and/or Local Contacts. <br />2. Provide a list of any subcontractors, their business address, and a summary <br />of their role in the project. <br />3.
