REV: 11-26-25 MI <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Consultant will perform all services onsite at a City facility. Consultant will perform <br />an initial assessment of all sworn Fire Department employees, including <br />firefighters, fire captains, etc. Their first session will include: Functional Movement <br />Screening, Range of Motion, and Strength Testing. Consultant will use the <br />information from the initial assessment to customize a treatment plan for each <br />individual. Consultant shall provide physical therapy treatment services on an as- <br />needed basis to the City. The City will be invoiced on an hourly basis for actual <br />services provided. <br />Consultant shall perform the services outlined above on an hourly basis at the rate <br />below.
