Consultant <br />will provide clearance forms for each participant. The Annual Firefighter Physical Exam <br />cost is $810 per person. Up to ninety-seven (97) employees will undergo testing. <br />Consultant will bill for services rendered. <br />The Annual Firefighter Exam Includes: <br /> <br /> Physician Examination <br /> <br /> Cardiovascular Evaluation <br /> <br /> Resting Vitals and Ancillary Testing <br /> <br /> Body Measurements, Spirometry, Hearing, & Vision Testing <br /> <br /> Comprehensive Metabolic Profile <br /> <br /> DL546 (Firefighter endorsement paperwork) <br /> <br />Additional Required Test Add-on <br /> <br />QuantiFERON-Tuberculosis Test. The cost is $120 per person (required for all <br />participants). <br />Additional Optional Test Add-ons <br /> <br />Serum, Testosterone Test. The cost is $50 per person (optional test for all males). <br />Thyroid Stimulating Hormone. The cost is $30 per person (optional for all females). <br /> <br />Other Potential Service Fees (If Necessary): <br /> <br />Chest X-Ray. The cost is $90 per person (only used if medically necessary) <br />Commercials driver’s license fee (only if necessary). The cost is $50 per person.
