Virginia) <br />Assessor's Parcel No.: 069-092-040 <br />Property Owner: OM Investments LLC Phone No.: (703) 400-2488 <br />Designated Contact: Nikko Kandhari Phone No.: (703) 400-2488 <br />Mailing Address: 1312 Laurel Street Suite 102 San Carlos CA 94070 <br />The property contains one pervious pavement area, located and sized as described below and as shown in the <br />attached site plan'. <br />Square feet of area <br />Pervious Pavement Area No. 1 is located at the site driveway 417 square feet <br />Routine Maintenance Activities <br />Types of pervious pavement include pervious concrete, porous asphalt, pervious pavers, permeable pavers, <br />and reinforced grid paving. The principal maintenance objective is to prevent sediment buildup and clogging, <br />which reduces infiltration capacity and pollutant removal efficiency. Routine maintenance activities, and the <br />frequency at which they will be conducted, are shown in Table 1. <br />Table 1 <br />Routine Maintenance Activities for Pervious Pavement Areas <br />No. <br />Maintenance Task <br />Frequency of Task <br />1 <br />Check for sediment and debris accumulation. Replace missing joint filler between pavers. <br />4 <br />Check for standing water on the pavement surface within 30 minutes after a <br />Two to four times annually <br />storm event. <br />5 <br />Inspect underdrain outlets and cleanouts, preferably before the wet season. <br />Two to fourtimes annually <br />Remove trash/debris. <br />6 <br />Remove sediment and debris accumulation on pervious pavement. <br />Two to four times annually <br />7 <br />Remove weeds. Mow vegetation on reinforced grid paving (such as turf <br />As needed <br />block) as needed. <br />8 <br />Perform restorative surface cleaning with a vacuum sweeper, and/or <br />As needed <br />reconstruction of part of the pervious surface to restore surface <br />permeability as needed.
