REV: 03-20-26 LF <br />EXHIBIT “A” <br />Program Objectives, Scope of Services & Quantitative Goals <br />Program Year 2025/2026 <br />Subrecipient Information <br />Subrecipient Name:Rebuilding Together Peninsula <br />Address: 841 Kaynyne Street <br />City, State, ZIP: Redwood City, CA 94063-3033 <br />Project Name: Safe at Home 2025-26 <br />Program Objectives <br />Project will address Redwood City’s Consolidated Plan 2023-2027 priorities <br />and goals as follows: <br />Priority: Creating Inclusive Communities with Affordable Housing - Creation <br />of affordable housing and preservation of existing affordable housing, including <br />special needs housing. <br />Goal 2 - Affordable Housing Preservation: Conserve and improve the <br />condition of the existing affordable housing stock and preserve affordable <br />housing developments at risk of conversion to market rate housing. <br />Scope of Services <br />Description of Services to be Provided: <br />In accordance with the terms of this Agreement and the joint County and Cities <br />Minor Home Repair Guidelines, Rebuilding Together will provide minor home <br />repairs for low-income Redwood City elderly and disabled households. <br />Project Activities: <br />Provide minor home repairs for low-income Redwood City elderly and disabled <br />households. <br />Primary Quantitative Goals: <br />Safe at Home 2025-26 will serve 5 Redwood City households. <br /> <br />NOTE: Performance will be measured by program objectives and quantitative goals.
