REV: 03-06-26 LF <br />EXHIBIT “A” <br />Program Objectives, Scope of Services & Quantitative Goals <br />Program Year 2025/2026 <br />Subrecipient Information <br />Subrecipient Name:Samaritan House <br />Address: 295 N Access Road <br />City, State, ZIP: South San Francisco, CA 94080 <br />Project Name: Safe Harbor Shelter <br />Program Objectives <br />Project will address Redwood City’s Consolidated Plan 2023-2027 <br />priorities and goals as follows: <br />Priority: Supporting Persons Experiencing Homelessness – Making <br />homelessness short, rare and non-reoccurring by supporting the work of <br />emergency shelters and funding other basic needs. <br />Goal 4 - Homeless Services: Provide high-quality, professional services <br />in all temporary shelters and comprehensive outreach services designed <br />to directly connect individuals to all appropriate housing and services. <br />Scope of Services <br />Description of Services to be Provided: <br />Safe Harbor Shelter will provide short-term, emergency housing with supportive <br />services to 112 unhoused adults from Redwood City. <br />Project Activities: <br />Safe Harbor Shelter will provide emergency housing and supportive services for <br />112 residents from the City of Redwood City. Supportive services include the <br />development of an individualized housing and service plan or individualized <br />empowerment plan focused on: (1) housing readiness measures, (2) housing <br />stability measures, and (3) the development of ongoing housing-related goals that <br />emphasize not only the acquisition of housing but ongoing housing stability. <br />Clients will also receive referrals to physical and mental health services, <br />employment assistance programs, housing agencies, educational programs, <br />legal aid services and more. <br />Primary Quantitative Goals: <br />Safe Harbor Shelter will serve 112 Redwood City persons. <br />NOTE: Performance will be measured by program objectives and quantitative goals.
