<br /> PART II, Page | 37 <br /> <br />Mandatory Attachments for RFP Submission <br />The following five attachments must be uploaded into the BSCC Submittable <br />Application Portal when prompted. BLANK ATTACHMENTS WILL NOT BE <br />ACCEPTED. <br />All of these attachments can be downloaded from the Prop 64 Grant homepage. <br />1. Project Budget Attachment <br />See “Project Budget Instructions” on preceding page, and sample “Project Budget” in <br />Appendix I. <br />2. Letter of Eligibility <br />Applicants must provide a Letter of Eligibility detailing how the city or county meets <br />the eligibility criteria contained in the Revenue and Taxation Code, § 34019, subd. <br />(f)(3)(C). <br />There is no required format for the Letter of Eligibility. An applicant may either use the <br />template provided on the Prop 64 Grant homepage or create their own (see <br />Appendix A as a sample). However, if an applicant creates their own letter, it must: <br />✓ Be on the county or city letterhead. <br />✓ Include the titles of the regulations, ordinances, or amendments to local <br />government charters. <br />✓ Include the dates the regulations, ordinances, or amendments to local <br />government charters became enacted/effective. <br />3. Certification of Compliance with BSCC Policies on Debarment, Fraud, Theft and <br />Embezzlement <br />Please see Appendix E as a sample. This form is available online and must be <br />completed, signed, and uploaded into the BSCC Submittable Application Portal. <br />4. Criteria and Assurance for Non-Governmental Organizations that Receive <br />BSCC Grant Funds as a Subcontractor <br />Please see Appendix F as a sample. This form is available online and must be <br />completed, signed, and uploaded into the BSCC Submittable Application Portal. <br />5. Project Work Plan <br />Please see Appendix G as a sample. This form is available online and must be <br />completed and uploaded into the BSCC Submittable Application Portal. <br />*Governing Board Resolution (optional at time of submission) <br />Please see Appendix D for sample language. An entity’s governing board’s resolution is <br />optional for upload at the time of application but must be submitted before the grant <br />agreement can be executed. <br />7.C. - Page 45 of 101 <br />104
