REV: 04-14-26 LF <br />EXHIBIT “A” <br />Program Objectives, Scope of Services & Quantitative Goals <br />Program Year 2025/2026 <br />Subrecipient Information <br />Subrecipient Name:Mental Health Association Of San Mateo County <br />Address: 2686 Spring Street <br />City, State, ZIP: Redwood City, CA 94063 <br />Project Name: Spring Street Shelter <br />Program Objectives <br />Project will address Redwood City’s Consolidated Plan 2023-2027 <br />priorities and goals as follows: <br />Priority: Supporting Persons Experiencing Homelessness - Making <br />homelessness short, rare and non-reoccurring by supporting the work of <br />emergency shelters and funding other basic needs. <br />Goal 4 - Homeless Services: Provide high-quality, professional services <br />in all temporary shelters and comprehensive outreach services designed <br />to directly connect individuals to all appropriate housing and services. <br />Scope of Services <br />Description of Services to be Provided: <br />Spring Street Shelter provides emergency shelter for individuals with serious <br />disabling mental illness <br />Project Activities: <br />Spring Street Shelter will provide shelter and support services to 57 Redwood <br />City residents <br />Primary Quantitative Goals: <br />Spring Street Shelter will serve 57 Redwood City persons. <br /> <br />NOTE: Performance will be measured by program objectives and quantitative goals.
