REV: 04-30-26 MI <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Consultant shall provide Fitness for Duty Examination services for City employees, <br />including sworn peace officers and firefighters, as requested by the City on an as-needed <br />basis. Services shall include: <br />1. Review of documents and materials provided by the City relevant to the <br />examination being requested. <br />2. Psychological testing and clinical interview of the subject employee. <br />3. Preparation of a written report/letter in accordance with the California <br />Confidentiality of Medical Information Act (Civil Code Section 56.10), addressed to <br />the City-designated representative, describing whether the individual may continue <br />to perform their normal and customary duties. <br />FEE <br />Consultant shall be compensated at a rate of $300.00 per hour for services performed <br />under this agreement. <br />KEY PERSONNEL <br />The following individual is designated as key personnel for purposes of this Agreement: <br />Griselda Lloyd, Licensed Marriage and Family Therapist <br />ATTY/AGR.2026.118/Core Mental Health Services (Agreement for Services) (Page 11 of 12)
