The Vote Center location shall not be changed without written <br />authorization from a Supervisor from the San Mateo County Registration & Elections Division <br />(“Division”), the Assistant Chief Elections Officer or the Chief Elections Officer (“Authorized Staff”). <br />No other event shall take place in the designated voting area for the duration of the use period. <br />Payment for the use of Facility shall be as indicated below: <br />Facility agrees to waive payment. <br />Full payment required. $ per day that the Facility is used by the Division. <br />(Facility must provide W-9) <br />Full payment required. $ total for all days that the Facility is used by the <br />Division. (Facility must provide W-9) <br />Partial payment. $ per day for the following days: <br />. <br />(Facility must provide W-9) <br />If payment is not waived, the Facility shall provide an invoice to the Division no later than November 24, <br />2026, or five business days following the execution of this Agreement, whichever is later. The Facility <br />understands that the County of San Mateo cannot be charged prior to Election Day. <br />No amendment to this Agreement will be effective unless it is agreed to by Authorized Staff. <br />The Facility agrees to indemnify, defend, and hold harmless the County of San Mateo, and its agents, <br />representatives, servants, contractors, employees, volunteers, successors, and assigns from and <br />against any and all liability, claims, damages, and causes of action of any kind whatsoever, caused or <br />alleged to be caused by the Facility and/or its agents, representatives, servants, contractors, <br />employees, successors, and assignees involving use of the Facility as a Vote Center. <br />ATTY/AGR.2026.200/San Mateo County Elections Office (Downtown Library_4-Day Vote Center Use Agreement N)(REV: 07-10-26 VR) (Page 7 of 8)
