<br />EXHIBIT A <br /> <br />CITY OF REDWOOD CITY <br /> <br />OMBUDSMAN SERVICES OF SAN MATEO COUNTY, INC. <br /> <br />FISCAL YEAR 2009/2010 <br /> <br />CONTRACT OBJECTIVES <br /> <br />Quantifiable Goals to be Provided <br />With Requested Funds <br />To provide access to advocacy services <br />for all 858 residents in City of Redwood <br />Cit throu h monitorin facilities <br />To investigate and close 85 cases in City <br />of Redwood City involving unduplicated <br />individuals. We anticipate resolving 80% <br />of those to the satisfaction or partial <br />satisfaction of the resident or the <br />resident's responsible party <br /> <br />To train 10 new volunteers and increase <br />the volunteer base by 3 <br /> <br />Performance Indicators (how you will <br />measure the quantifiable oal) <br />The facility coverage plan is a measure of how <br />many facilities have been visited and how often <br /> <br />As complaints are received they are entered in <br />a database, given a case number and tracked <br />through closure. One of the fields that must be <br />addressed is the disposition of the case, and <br />one of the possible dispositions is "resolved to <br />the satisfaction/partial satisfaction of the <br />resident." Once the data has been entered, <br />reports are generated which give us the data <br />needed to determine if we are meetin oals <br />Currently we have 36 active volunteers and 6 <br />inactive. A count of state-certified volunteers <br />working in the program will give us the <br />information needed to know if we are meeting <br />the established oals <br /> <br /># of Unduplicated RWC Residents/Households to be Assisted (Only programs serving <br />individuals ma re ort b individuals. All others should report b households) <br />858 individuals will be served -- this number will change based on when Sequoia Hospital <br />DP-SNF closes. <br />
