<br />Healthcare Coverage. The grantee (Public Allies) must provide healthcare <br />insurance to those members serving on a full-time basis who are not otherwise <br />covered by a healthcare policy at the time each begins his/her period of full-time <br />service. For purposes of this provision, a member is serving on a full-time basis <br />when his/her regular term of service will involve performing service on a normal full- <br />time schedule for a period of six weeks or more. A member may be serving on a full- <br />time basis without regard to whether his/her agreed term of service will result in a <br />full-time Segal AmeriCorps Education Award. The grantee must also provide <br />healthcare insurance to members serving on a full-time basis who lose coverage <br />during their term of service as a result of service or through no deliberate act of their <br />own. The Corporation will not cover healthcare costs for family members or for <br />members who have not served on a full-time, or who have ceased serving on a full- <br />time basis. <br /> <br />Minimum Benefits. When required to provide healthcare insurance coverage, the <br />grantee may obtain healthcare from any provider as long as the coverage provided <br />by the grantee provides the following minimum benefits: <br /> <br />a. Physician services for illness or injury <br /> <br />b. Hospital room and board <br /> <br />c. Emergency room <br /> <br />d. X-ray and laboratory <br /> <br />e. Prescription drugs <br /> <br />f. Limited mental/nervous disorders <br /> <br />g. Limited substance abuse coverage <br /> <br />h. An annual deductible of no more than $250 charges per member <br /> <br />I. No more than $1,000 total annual out-of-pocket per member <br /> <br />J. A 200/0 co-payor a comparable fixed fee with the exception of a 500/0 <br />co-pay for mental and substance abuse care <br /> <br />k.
