Ackerman <br /> <br />Task 1: Monthly Monitoring and Quarterly Reporting $22,800 <br />GEl will conduct monitoring of the treatment system from January 2010 through December 2010. GEl <br />will collect groundwater samples for subsequent laboratory analyses as per the attached table. <br />GEl will prepare a draft Quarterly Monitoring Report as per NPDES permit requirements, for the City's <br />review, prior to report finalization and submittal to the RWQCB. The report will be uploaded to <br />Geotracker in pdf format. GEl will provide a pdf version of each final report to the City. Each report will <br />include a letter of transmittal, a compliance evaluation summary, a location map, the results of analyses <br />and observations, data on flow and mass removed, and operational status. The fourth quarter report will <br />be an Annual Report covering the previous calendar year. In addition to the quarterly report <br />requirements, the annual report will contain a comprehensive discussion of the compliance record and <br />any corrective actions necessary. To be billed monthly in equal increments of $1,900. <br /> <br />Task 2: Additional sampling as required due to operational issues or as requested by the City or <br />RWQCB. Changeout of particulate filters, attendance at meetings, or other tasks as requested by the $3,000 <br />City or RWQCB. To be billed on a time and material basis as per the attached 2009 Schedule of <br />Charges, with a Not-to-Exceed cost of $3,000 <br /> <br />TOTAL LUMP SUM D or ESTIMATED [gJ PRICE $25,800 <br /> <br />Agreement: The Agreement between GEl and Customer will consist of this executed Proposal, and The General Terms and Conditions <br />of the Agreement between GEl and the Customer (General Terms).
