DUPLICATE ORIGINAL MEMORANDUM OF UNDERSTANDING BElWEEN Samaritan House and Fair Oaks Community Center FOR ASSISTANCE WITH COUNTY OF SAN MATEO FUNDED HOMELESSNESS PREVENTION AND RAPID RE..HoUSING (HPRP) SERVICES Samaritan House_and Fair Oaks Community Center agree to collaborate for the purpose of implementing the County of San Mateo Funded Homeless Prevention Services designed to bolster housing stabilization and provide temporary financial assistance and/or services to help persons gain housing stability in the County of San Mateo. This agreement is wholly dependent on funding from the County of San Mateo Housing and Community Development (HCD) Federal Stimulus Funds under the American Recovery and Reinvestment Act of 2009 (ARRA). Any shortfall from the County of San Mateo funding will result in a corollary shortfall in payment from Samaritan House to Fair Oaks Community Center. The Fair Oaks Community Center will hereinafter be called "Contractor" for this agreement. Scope of Services to be provided CONTRACTOR agrees to provide: 1) Homeless Prevention Services a) Financial Assistance b) Housing Stabilization and Relocation Services All service activities to be in accordance with Exhibit A - Scope of Services. Contractor will hold primary responsibility for the design and implementation of its programs and will provide Samaritan House with any updates concerning changes in service availability. As needed.1 based on future program evaluation findings and/or funding availability, the scope and design of services may be modified by joint agreement between Samaritan House and Contractor. If Contractor fails to provide services as scheduled or if service provision is significantly delayed or interrupted, Samaritan House may terminate this agreement with a minimum of 30 days notice to Contractor. All services will be provided as a component of the larger HPRP collaborative, for which the program oversight committee (described below) will collaborate on coordination of services, resources and objectives. If, in collaboration with the oversight committee, Samaritan House determines in the future that Contractor's services no longer meet program objectives, or if funding availability dictates that these services must be reduced or eliminated, this agreement may be terminated by Samaritan House with a minimum of 30 days notice to Contractor. Individualized Housing and Service Plan documents Contractor agrees to use and complete all forms and requested data collection required by Samaritan House and the County of San Mateo HCD related to this project. The basic components of the data collection includes, but not limited to, the following: Initial Screening Questionnaire, Risk Determination Assessment, Client Self Declaration of Housing Status, Client Self Declaration of Income, Verification of Income, Homeless Certification, HPRP Staff Affidavit Form, Fair Oaks Community Center - COUNTY HCD HPRP funding; November 1, 2009-September 30, 2011
