<br />Step 2: Problem solving with Wellness Manager andl or RWC 2020 <br />Staff members should bring all issues of concern to the attention of their direct supervisor for <br />initial discussion and problem solving. If the staff member is an employee of FFY, the supervisor <br />should notify the Wellness Manager of the existence of these issues of concern and the steps <br />being taken to resolve them. If resolution is not reached, agency leadership should be involved. <br /> <br />Step 3: Involvement of agency Directors <br />If these previous steps do not result in the resolution of concerns, RWC 2020's Executive Director <br />and the Executive Director of FFY should work together with involved staff members to address <br />and resolve the problem. <br /> <br />If the prior steps do not result in the resolution of concerns, San Mateo County BHRS staff will be <br />available for support. At any time in this process, any involved party should feel free to request <br />the involvement of a neutral third party to facilitate communication and problem solving. FFY <br />and/or RWC 2020 will provide staff members with information about how to access the services <br />of a neutral third party. <br /> <br />ACCOUNT ABILITY <br /> <br />RWC 2020 and FFY will meet with the appropriate staff at the beginning of the project to establish <br />a work plan for Prevention Partnership related services. This work plan will include FFY reporting <br />and data collection responsibilities for the year. RWC 2020 and/or the Community Wellness <br />Manager and FFY will meet on a quarterly basis to review the work plan, address changes, <br />resolve problems and address any professional development/training needs for the community. <br /> <br />During the term of this contract, RWC 2020/Community Wellness Manager will conduct site visits <br />to observe activities/events as appropriate. <br /> <br />PA YMENTS: <br />FFY will submit to Redwood City 2020 a detailed invoice for payment by May 1, 2010 for $6,000 <br />for specified services, to support the community-based AOD prevention effort. <br />See Attachment A. <br /> <br />RWC 2020 will approve the invoice within ten (10) business days after the invoice is received or, <br />if there are questions or concerns, notify FFY within that period. <br /> <br />REPORTING GUIDELINES: <br /> <br />FFY will submit bi-weekly data reports to RWC 2020 on the 2nd and 4th Wednesday of every <br />month. This report will reflect all activities described in the attached Scope of Work and work <br />plan, as well as the number of clients and demographics of all populations served. See <br />Attachment B. Payment for services is continqent on timely submission of bi-weekly reports. <br /> <br />AVAILABILITY OF FUNDS: <br />RWC 2020 may terminate this Understanding or a portion of the services referenced in the <br />Attachments due to unavailability of Federal, State, or County (BHRS) funds, by providing written <br />notice to FFY as soon as is reasonably possible after learning about this unavailability of outside <br />funding. <br /> <br />FUTURE YEAR PLANNING <br />Discussion of service implementation for future years will commence before the end of the 2009- <br />2010 Fiscal year. <br /> <br />RDA Agreement under $1 OK <br />City Attorney Approved Version 012710 <br /> <br />5 <br />
