That certain application entitled, "Application for <br />Elective Coverage of Disability Insurance Only - Local Public <br />Entities" (form, "DE 1378M Rev. 3 (7-86)"), pursuant to which <br />application is made to the Director of Employment Development of <br />the State of California for disability insurance purposes, only, <br />pursuant to Section 709 of the California Unemployment Insurance <br />Code, a copy of which application is on file in ~he <br />Port administrative offices, to which copy reference is hereby <br />made for {he full particulars thereof, is hereby approved, and <br />the Executive Director is hereby authorized and directed to file <br />said application with said Director of Employment Development. <br /> <br /> 3.
