EXHIBIT B <br /> Quantitative Goals <br /> (Contract Objectives) <br /> Shelter Network <br /> 1450 Chapin Avenue, 2 Floor <br /> Burlingame, CA 94010 <br /> Activity /Program Title: Maple Street Shelter <br /> FISCAL YEAR 2010/2011 <br /> OBJECTIVE AND STRATEGIES NUMBER OF PERSONS COMPLETION <br /> OR HOUSEHOLDS DATE <br /> Primary quantitative contract goal' (Include only ONE primary goal in this section). <br /> HOM 2: Support existing emergency shelter 250 individuals 6/30/11 <br /> facilities and programs. (Provide safe shelter, basic <br /> necessities, and targeted supportive services for <br /> individuals who would otherwise be living on the <br /> street or in other unsafe situations.) <br /> Other secondary goals, collaborative actions, outreach, and emphasis to be conducted. <br /> HOM 4: Meet the human services needs of the 109 individuals 6/30/11 <br /> homeless. (Provide ongoing support, through <br /> weekly case meetings and workshops, for clients to <br /> improve their finance and budgeting skills and to <br /> access job search and development resources, so <br /> that participants' median income will increase by at <br /> least 20% between program entrance and exit.) <br /> HOM 5: Prevent homelessness. (Provide 76 individuals 6/30/11 <br /> individuals in the short-term shelter program with <br /> assessments, targeted case management, <br /> information, referrals, and on -site services on an <br /> ongoing basis, to enable at least 70% of clients <br /> completing the program to move on to more stable <br /> or permanent housing.) <br /> HOM 5: Prevent homelessness. (Provide 53 individuals 6/30/11 <br /> individuals in the mid -term transitional housing <br /> program with assessments, targeted case <br /> management, information, referrals, and on -site <br /> services on an ongoing basis, to enable at least <br /> 75% of clients completing the program to return to <br /> permanent homes of their own.) <br /> 1 Primary quantitative contract goal is the number of households OR persons you will assist under this <br /> contract as stated in your grant application.
