611 B -q <br />Waterworks Industries inc. will be on call 24 hours a day, and at least one of the <br />three key people will be available at all times. During the project. Waterworks <br />Industries, Inc. personnel will call Rich Del Ben, Utilities Specialist, and apprise him <br />of the time we are performing the Water Quality Monitoring. The monitoring usually <br />occurs on Wednesday. A Water Quality Monitoring report is prepared and faxed to <br />Redwood City Public Works Services Department on the following Monday. The <br />report is accompanied by any Chemical Application or Harvesting that was <br />performed. <br />As per the Lagoon Management Plan, Waterworks Industries will make public <br />appearances to Home Owners Associations and Redwood Shores Residents groups to <br />inform and answer questions regarding the Lagoon and its management. Waterworks <br />Industries, Inc. will attend up to three meetings at no cost to Redwood City. <br />Additionally, Waterworks Personnel can better answer some questions that Staff gets <br />from the public. Phone calls from residents are welcome, provided Redwood City <br />Staff referred them to us, this also at no additional cost <br />If there is dissatisfaction expressed by Redwood Shores residents or Redwood City <br />staff with regards to Waterworks Industries, Inc. performance of the Lagoon <br />Management Services, a phone call, followed up with a fax from Redwood City's <br />Staff, to Richard Carnation, President and Redwood Shores Project Supervisor will be <br />responded to immediately by phone. Further, confirmation of dissatisfaction and a <br />resolution program will be faxed immediately back to Redwood City Staff and the <br />program will be implemented. <br />4. VERIFICATION OF INSURANCE <br />Waterworks Industries, Inc. shall keep on file with the City of Redwood City a current <br />Certificate of Insurance meeting all the requirements for General liability, <br />Automobile and Workers Compensation. Upon award of the contract and / or <br />subsequent Task Order, which extend the term of the Agreement, Waterworks <br />Industries, Inc. will provide current certificates. <br />6 of 7 <br />Waterworks Industries, Inc.
