6.1 D <br /> Page 4 <br /> AGREEMENT BETINEEN THE COUNTY �F SAN MATEO <br /> AND <br /> The Cify of Redwood City Fair Oaks Commun�ty Center <br /> THIS AGREEMENT, entered into this day of , 20 , by <br /> and between the COUNTY OF SAN MATEO, hereinafter called "County," and The C1ty of <br /> Redwood City Fair Oaks Community Center, hereinafter called "Contractor"; <br /> WITNESSETH: <br /> WHEREAS, Contractor applied for funding assistance for The Fair Oaks Community ' <br /> Center Information and Referral Program ; and <br /> WHEREAS, on April 27, 2010, the Board of Supervisors approved the CDBG/ESG/ <br /> H�ME Action Plan funding for FY 2010-11, which included funding for this activity; and <br /> WHEREAS, it is necessary and desirable that Contractor receive funding assistance for <br /> the purpose of providing vital�safety net services to low-income families, single parents, <br /> seniors and individuals seeking assistance in meeting their basic human needs; <br /> NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOI.LOWS: <br /> 1. Exhibits <br /> The following exhibits and attachments are included hereto and incorporated by reference <br /> herein: <br /> Exhibit A - Program/Project Description <br /> Exhibit B- Disbursements and Rates <br /> Exhibit C- Contractor Declaration Form <br /> Exhibit D- Additional Program Requirements <br /> Exhibit E - 504 Compliance <br /> Notwithstanding any other provisions of this Agreement, in the event of a conflict befiween <br /> the terms of this Agreement and fihose of an Exhibit, the terms of fhe Exhibit will control. <br /> 2. Services to be performed by Contractor <br /> In consideration for the funding assistance set forfh herein and in Exhibit "B," Contractor shall <br /> perform the senrices (hereinafter referred to as the "services" or the "work") necessary to <br /> implement fihe Program/Project as described in Exhibit A. For the purposes of this contract <br /> "Program" or "Project" mqy be used interchangedbly. <br /> 3. Payments <br /> In considerc�tion of the services provided by Contractor in accordance with all terms, <br /> condifiions and specifications set forth herein, County shall disburse funding assistance to <br /> Contractor in the form of payments based on the rates and in the manner specified in <br /> Exhibit "B."
