6.1 D <br /> Page 12 <br /> Exhtbit A <br /> Program/Project Description and Specific Requirements <br /> . . <br /> Program Description. <br /> In consideration of the funding assistance payments set forfh in Exhibit B, Contractor <br /> shall provide the following services: Provide vital sa#ety net services to low-income families, <br /> single parents, seniors, disabled persons and individuals seeking assistance in meeting their <br /> basic human needs. To provide appropriate and immediate emergency assistance <br /> including homeless assistance to families and single adults in the form of direct service <br /> provision or referrals to other service providers. To prevent homelessness by providing <br /> emergency and other services to families and individuals who are at risk of becoming <br /> ' homeless. � <br /> Goal: To serve 92 qualified low-income families. These 92 families are parf of Contractor's <br /> estimated 550 low- income families to be served from Unincorporated Redwood City (Norfh <br /> Fair Oaks) and other qualified geographic areas of the County. <br /> Countv Reauirements <br /> 1. County Reimbursement Pctymenfi �verview: CDBG funds may not be used as advances <br /> to Contracfior, only for reimbursement for services rendered. CDBG may be used to pay <br /> for costs incurred for fihe following: the cost of labor, supplies, and moterials to deliver <br /> the above-referenced service(s�, and a share of costs of operating and maintaining a <br /> portion of the facility in which the service(sj is/are located. This includes the lease of a <br /> facility, equipment, and ofiher property needed for the public service. Payment to <br /> Contractor may be made based on a unit cost of service delivery. <br /> CDBG will not pay for income payments to individuals for such items as food, clothing, <br /> rent, and utilities. lncome payments made on behalf of an individual to providers of <br /> such services ond not exceeding three consecutive months would be an eligible CDBG <br /> cost. CDBG fiunds may not be used to support Contractor administration, or other costs <br /> not associated with the CDBG-funded service delivery. Costs must be allowable, <br /> reasonable, and allocable, subject to �MB Circular A-122, "Cost Principles for Non-Profit <br /> Organizations", or OMB Circular No. A-87, "Principles for Determining Costs Applicable to <br /> Grants and Contracts with Stdte, Local and Federally Recognized Indian Tribal <br /> Governments. <br /> Unless otherwise noted, payment to Contractor shall be made quarterly in accordance <br /> with the cost allocation budget below, which shows an estimated reimbursement <br /> schedule of amounts for County funding under this agreement. All invoices musfi be <br /> supported by adequate documentcttion acceptable to DOH. (See Exhibit B for more <br /> information.) Both County and Contractar understand that the cost allocation portion <br /> for Counfy reimbursement is an estimate, and may deviate from what has been sfiated <br /> below. Any line item request for reimbursement that varies greater than 10% from the <br /> original estimated CDBG-budgeted line item must be approved in writing by DOH <br /> Director. <br /> DOH Rev.
