� <br /> 6.1B •� '' <br /> Page 15 <br /> EXHIBIT B <br /> SAMARITAN HOUSE <br /> HPRP Client Records Confdentiality Policy <br /> POLICY <br /> Samaritan House is committed to respecting the confidentiality of the individuals and families receiving <br /> Homeless Prevention and Rapid Re-housing funds. Data collection is a requirement to rneet eligibility <br /> requirements in order to received funding under the American Recovery and Reinvestr�ent Act <br /> Homelessness Prevention and Rapid Re-Housing funds. <br /> The privacy of HPRP clients will be observed through agency and partner adherence to the following set <br /> of procedures: <br /> I. Clients receiving HPRP funding streams will be assigned a unique client identifier from the lead <br /> agency. This identifier will be documented in the client paper files and used in the Samaritan House <br /> HpRP access database; <br /> 2. Clients will automatically be assigned a client number when they are entered into San Mateo County's <br /> HMIS database. This number is generated automatically from the County HMIS system. The HMIS <br /> number will be documented in the paper client files as well as the access database. This means that <br /> clients will have two unique identifiers; <br /> 3. Samaritan House will maintain paper records for 100°!0 of the clients receiving HPRP funds from the <br /> State and County. These records will be maintained in a locked filing cabinet that is housed in a <br /> locked of�ce within a password protected office building; <br /> 4. Samaritan House will enter client data into the County-operated HMIS website, which is password <br /> protected and housed offsite; <br /> 5. Samaritan House will enter client data into the Samaritan House HPRP database, which is housed on <br /> the password protected Samaritan House intranet system; <br /> 6. Client information will not be shared with unauthorized third parties; <br /> 7. Clients will be informed of this Confidentiality Policy upon enuy into all State and County HPRP <br /> funded programs� <br /> 8.
