SINGLE - FAMILY MISSED PICK -UP INITIAL COMPLAINTS <br /> Incentive Payment for Exceptionally Low Missed Pick -Up Initial Complaint Level <br /> Contractor shall receive an incentive payment if the percentage of Missed Pick -Up Initial <br /> Complaints for Agency is less than one (1) per three - thousand (1:3,000) of the total <br /> Single- Family Service Opportunities during the monthly reporting period (e.g., 15,000 <br /> SFD that are provided service for three carts or 45,000 total weekly Service <br /> Opportunities x 4.33 weeks per month = 194,850 Service Opportunities per month, <br /> which equates to a total allowance of 58 or fewer Missed Pick -Up Initial Complaints each <br /> month; or, the total number of Complaints is less 0.033% of all Service Opportunities). <br /> In such cases, the incentive payment shall be calculated based on the following <br /> example: <br /> Actual number of Missed Pick -Up Initial Complaints = 36 <br /> Allowable threshold of Missed Pick -Up Initial Complaints = 58 <br /> Incentive Payment = $50 per complaint <br /> Incentive = Actual number < 0.033% of monthly Service Opportunities (i.e., 58) <br /> 36 — 58 = -22 = <58 <br /> 22 x $50 = $1,100 <br /> If the percentage of Single - Family Missed Pick -Up Initial Complaints for Agency is equal <br /> to or greater than one- per three - thousand (1:3,000) Service Opportunities during the <br /> monthly reporting period, no incentive payment shall be provided for the monthly <br /> reporting period. <br /> Franchise Agreement for Collection Services with Recology San Mateo County Attachment I <br /> Redwood City Performance Incentives and Disincentives <br /> Page 9 of 13 <br />
