Agenda development, logistical coordination, notes and next steps <br /> • Facilitate 4 -6 Community Meetings /gatherings /events <br /> a. Provide coordination, planning, facilitation, follow -up, and logistics for <br /> community engagement events at least once a Quarter <br /> • Facilitate connection between PCRC resources, specifically but not limited to <br /> Family Engagement staff and Pacific Islander Community. <br /> • Ensure ongoing communication with contracted and non - contracted partners with <br /> monthly in person or phone conversations <br /> • Provide 2 -3 1 on1 meetings with each individual contracted partner identifying <br /> milestones, solidifying contract agreements and conducting year end reviews <br /> • Ensure appropriate County reporting and data collection requirements are met <br /> including but not limited to attending monthly Provider meetings, entering and <br /> maintaining Prevention Partnership monthly CaIOMS records, Quarterly Reports <br /> and Annual Reports. <br /> • Document efforts of Prevention Partnership during grant year to track indicators. <br /> • Support Executive Director of RC 2020 to identify and develop appropriate <br /> evaluation strategies and tools <br /> • Work collaboratively with RC 2020 Executive Director in establishing universal <br /> messaging and marketing materials by December 2010. <br /> • Meet Prevention Partnership Expectations outlined in Exhibit "C" <br /> Agreement over $10K 10 <br /> City Attorney Approved Version 041410 <br />
